Agreement to Terms
Welcome to IV Wellness Nurse Inc. (“Company,” “we,” “us,” or “our”). These Terms of Service (“Terms”) govern your access to and use of our website at www.ivwellnessnurse.com, our mobile IV hydration and vitamin therapy services, and any related communications, scheduling, or interactions with our business.
By accessing our website, scheduling an appointment, receiving any of our services, or otherwise engaging with IV Wellness Nurse Inc., you acknowledge that you have read, understood, and agree to be bound by these Terms. If you do not agree to these Terms, please do not use our website or services.
Description of Services
IV Wellness Nurse Inc. provides mobile and in-office IV hydration therapy, intramuscular (IM) vitamin and nutrient injections, and related wellness services. Our services are administered by licensed, experienced registered nurses under the supervision of a licensed medical director.
Services are available at our partner clinic locations, including Mira Med Spa in Valrico, FL and The Holistic Center for Vibrant Health in Oldsmar, FL, as well as through our mobile concierge service throughout the Tampa Bay area, including Tampa, Clearwater, St. Petersburg, and Orlando.
Our services are intended for general wellness purposes only and are not a substitute for professional medical advice, diagnosis, or treatment. You should always seek the advice of your physician or other qualified healthcare provider with any questions regarding a medical condition.
Eligibility
Our services are available to individuals who are 18 years of age or older. By using our services, you represent and warrant that you are at least 18 years old and have the legal capacity to enter into these Terms. Individuals under the age of 18 may receive services only with the written consent of a parent or legal guardian who agrees to these Terms on their behalf and is present during treatment.
Appointments and Scheduling
Appointments may be scheduled through our online scheduling platform (currently Acuity Scheduling), by phone at 706-294-5961, by text message, or by email at Ivwellnessnurseinc@gmail.com.
By scheduling an appointment, you agree to provide accurate and complete information, including your contact details, selected service, and any relevant health information. You also agree to arrive on time for in-office appointments or to be available at the designated location for mobile appointments.
Cancellation Policy. We require at least 24 hours’ notice for appointment cancellations or rescheduling. Cancellations made with less than 24 hours’ notice may be subject to a cancellation fee. No-shows may also be subject to a fee and may affect your ability to book future appointments.
Mobile Service Fees. Mobile IV services include an additional concierge fee, which will be communicated to you at the time of booking. Mobile appointments require a confirmed address within our service area.
Health Disclosures and Informed Consent
Prior to receiving any IV hydration or injection services, you will be required to complete an intake form disclosing your medical history, current medications, known allergies, and any other health information relevant to your treatment. You are responsible for providing accurate and complete health information.
You understand and acknowledge that IV hydration therapy and vitamin injections, like all medical procedures, carry certain risks, including but not limited to bruising, swelling, infection at the injection site, allergic reactions, vein irritation, and in rare cases, more serious complications.
By receiving services from IV Wellness Nurse Inc., you provide your informed consent to the administration of the treatment(s) you have selected, and you acknowledge that you have had the opportunity to ask questions and that your questions have been answered to your satisfaction.
If at any time during treatment you experience discomfort or an adverse reaction, you agree to immediately notify the treating nurse.
Payment and Pricing
Payment is due at the time of service unless otherwise arranged in advance. We accept major credit and debit cards and other payment methods as communicated at the time of booking.
Prices for services are listed on our website and are subject to change without prior notice. Any price changes will not affect appointments that have already been confirmed at a previously quoted price.
IV Wellness Nurse Inc. does not bill insurance. Our services are self-pay, and it is your responsibility to determine whether your insurance provider offers any reimbursement for the services you receive.
Communications and Text Messaging
By providing your phone number, you consent to receive communications from IV Wellness Nurse Inc. via phone call, text message (SMS/MMS), and email. These communications may include appointment confirmations, reminders, follow-up messages, service updates, and, where you have opted in, promotional messages.
Message frequency varies. Message and data rates may apply.
You may opt out of text messages at any time by replying STOP. You may opt out of marketing emails by following the unsubscribe link in any email. Opting out of promotional communications will not affect transactional or service-related messages related to your scheduled appointments.

Intellectual Property
All content on the IV Wellness Nurse Inc. website, including text, images, graphics, logos, and design elements, is the property of IV Wellness Nurse Inc. or its content suppliers and is protected by applicable intellectual property laws. You may not reproduce, distribute, modify, create derivative works from, or publicly display any content from our website without our prior written consent.
Limitation of Liability
To the fullest extent permitted by applicable law, IV Wellness Nurse Inc., its owners, officers, employees, nurses, contractors, and affiliates shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or related to your use of our website or services, regardless of the cause of action or the theory of liability.
Our total liability to you for any claim arising out of or related to these Terms or our services shall not exceed the total amount you paid to IV Wellness Nurse Inc. for the specific service giving rise to the claim.
Nothing in these Terms is intended to limit liability that cannot be limited under applicable law, including liability for gross negligence, willful misconduct, or violations of applicable healthcare regulations.
Indemnification
You agree to indemnify, defend, and hold harmless IV Wellness Nurse Inc., its owners, officers, employees, nurses, contractors, and affiliates from and against any and all claims, liabilities, damages, losses, costs, and expenses (including reasonable attorneys’ fees) arising out of or related to your use of our website or services, your violation of these Terms, your provision of inaccurate health information, or your violation of any applicable law or regulation.
Disclaimer of Warranties
Our website and services are provided on an “as is” and “as available” basis. IV Wellness Nurse Inc. makes no warranties, express or implied, regarding the operation of our website, the accuracy or completeness of any information provided, or the results of any treatment or service. We disclaim all warranties, including implied warranties of merchantability, fitness for a particular purpose, and non-infringement.
While our services are administered by licensed registered nurses under the supervision of a licensed medical director, individual results may vary, and we do not guarantee specific health outcomes from any treatment.
Governing Law and Dispute Resolution
These Terms shall be governed by and construed in accordance with the laws of the State of Florida, without regard to its conflict of law provisions. Any dispute arising out of or relating to these Terms or our services shall be resolved exclusively in the state or federal courts located in Hillsborough County, Florida, and you consent to the personal jurisdiction of such courts.
Severability
If any provision of these Terms is found to be invalid, illegal, or unenforceable by a court of competent jurisdiction, the remaining provisions shall continue in full force and effect. The invalid or unenforceable provision shall be modified to the minimum extent necessary to make it valid and enforceable while preserving the original intent of the provision.
Changes to These Terms
We reserve the right to update or modify these Terms at any time. When we make material changes, we will update the “Last Updated” date at the top of this page. Your continued use of our website or services after any changes to these Terms constitutes your acceptance of the revised Terms.
